Individual Membership Forms

Some individual members will return the following forms to Bookshare. Please register online before submitting these forms. You’ll be instructed during registration about which forms you’ll need to submit.

Volunteer Forms

Individuals wishing to volunteer with Bookshare must complete the following form. Please register online before submitting this form.

Publisher Forms

Publishers can grant permission to add books directly to the Bookshare library and provide international permissions by completing the following form:
