What do we mean when we talk about rights in relation to health? Where does the language of health rights come from, and what are the implications of using such a discourse? During the last 20 years there have been an increasing number of initiatives and efforts – for instance in relation to HIV/AIDS – which draw on the language, institutions and procedures of human rights in the field of global health. This book explores the historical, cultural and social context of public health activists’ increasing use of rights discourse and examines the problems it can entail in practice. Structured around three interlinked themes, this book begins by looking at what health as a right means for our understandings of citizenship and political subjectivities. It then goes on to look at how and why some health problems came to be framed as human rights issues. The final part of the book investigates what happens when health rights are put into practice – how these are implemented, realised, cited, ignored and resisted. Unlike many other areas in health care, the practice of oncology presents unique challenges that make assessing and improving value especially complex. First, patients and professionals feel a well-justified sense of urgency to treat for cure, and if cure is not possible, to extend life and reduce the burden of disease. Second, treatments are often both life sparing and highly toxic. Third, distinctive payment structures for cancer medicines are intertwined with practice. Fourth, providers often face tremendous pressure to apply the newest technologies to patients who fail to respond to established treatments, even when the evidence supporting those technologies is incomplete or uncertain, and providers may be reluctant to stop toxic treatments and move to palliation, even at the end of life. Finally, the newest and most novel treatments in oncology are among the most costly in medicine. This volume summarizes the results of a workshop that addressed these issues from multiple perspectives, including those of patients and patient advocates, providers, insurers, health care researchers, federal agencies, and industry. Its broad goal was to describe value in oncology in a complete and nuanced way, to better inform decisions regarding developing, evaluating, prescribing, and paying for cancer therapeutics.

A nuclear attack on a large U.S. city by terrorists--even with a low-yield improvised nuclear device (IND) of 10 kilotons or less--would cause a large number of deaths and severe injuries. The large number of injured from the detonation and radioactive fallout that would follow would be overwhelming for local emergency response and health care systems to rescue and treat, even assuming that these systems and their personnel were not themselves incapacitated by the event. The United States has been struggling for some time to address and plan for the threat of nuclear terrorism and other weapons of mass destruction that terrorists might obtain and use. The Department of Homeland Security recently contracted with the Institute of Medicine to hold a workshop, summarized in this volume, to assess medical preparedness for a nuclear detonation of up to 10 kilotons. This book provides a candid and sobering look at our current state of preparedness for an IND, and identifies several key areas in which we might begin to focus our national efforts in a way that will improve the overall level of preparedness.

Assisted Dying explores the law relating to euthanasia and assisted suicide, tracing its development from prohibition through to the laissez faire attitude adopted in a number of countries in the 21st Century. This book provides an in-depth critique of the arguments surrounding legislative control of such practices and particularly looks into the regulatory role of the state. In the classical tradition of libertarianism, the state is generally presumed to have a remit to intervene where an individual’s actions threaten another, rather than harm the individuals themselves. This arguably leaves a question mark over the state’s determined intervention, in the UK and elsewhere, into the private and highly personal choices of individuals to die rather than live. The perceived role of the state in safeguarding the moral values of the community and the need for third party involvement in assisted suicide and euthanasia could be thought to raise these practices to a different level. These considerations may be in direct conflict with the so called right to die espoused by some individuals and groups within the community. However this book will argue that the state’s interests are and should be second to the interests that the people themselves have in choosing their own death. Assisted Reproductive Technologies in the Global South and North critically analyses the political and social frameworks of Assisted Reproductive Technology (ART), and its impact in different countries. In the context of a worldwide social pressure to conceive – particularly for women – this collection explores the effect of the development of ARTs, growing globalisation and reproductive medicalization on global societies. Providing an overview of the issues surrounding ART both in the Global South and North, this book analyses ART inequalities, commonalities and specificities in various countries, regions and on the transnational scene. From a multidisciplinary perspective and drawing on multisite studies, it highlights some new issues relating to ART (e.g. egg freezing, surrogacy) and discusses some older issues regarding infertility and its medical treatment (e.g. in vitro fertilisation, childless stigmatisation and access to treatment).
